Total Quality Scores, measure points, cut points, the value per point and quality add-ons are Myers & Stauffer’s published Nursing Facility Total Quality Score Summary & Support workbooks for Indiana Medicaid. A period not yet published would be SuperLTC’s scoring of CMS’s public data on the same rules, and every page would say so. Operators are CMS’s chain names; map points are CMS’s geocodes. No patient data appears here.
